PART 6: RELEASE OF LIABILITY AND WAIVER OF CLAIMS
In consideration of being permitted to participate in AMISA programs, I — on behalf of myself, the Minor participant (if applicable), my heirs, assigns, executors, administrators, and legal representatives — hereby release, waive, discharge, and covenant not to sue:
• Academy for Martial and Intellectual Sciences and Arts, LLC (AMISA) and its owners, officers, directors, managers, instructors, staff, employees, agents, contractors, and volunteers;
• European Fitness Center, 2999 South Virginia Street, Reno, NV 89502, and its owners, operators, staff, and agents;
• All school-based partner sites, including Coral Academy and any other current or future partner school locations, and their respective administrators, staff, and employees;
• All other parties associated with AMISA program delivery at any location;
...from any and all claims, demands, actions, liabilities, damages, losses, costs, and expenses — including attorneys' fees — arising from or related to participation in AMISA programs, whether caused by the alleged negligence of any of the above parties or otherwise, to the fullest extent permitted by the laws of the State of Nevada.
I understand that this release covers claims for bodily injury (including death), property damage, and all other damages arising during or related to program participation.
NEVADA NOTICE: Some provisions of this release may be limited by Nevada law. This release applies to the maximum extent permitted by applicable law. Nothing herein waives liability for gross negligence or intentional misconduct to the extent such waiver is prohibited by law.
PART 7: INDEMNIFICATION
I agree to indemnify, defend, and hold harmless AMISA, European Fitness Center, all school-based partner sites, and their respective owners, operators, instructors, and staff from any claims, liabilities, damages, costs, and expenses (including reasonable attorneys' fees) arising from:
• My or the Minor participant's participation in AMISA programs
• My breach of this Waiver or AMISA's Terms of Service or program policies
• My or the Minor's misconduct, noncompliance with safety rules, or misuse of martial arts techniques
• Claims brought by third parties relating to my or the Minor's conduct
PART 8: EMERGENCY MEDICAL AUTHORIZATION
In the event of injury or illness occurring during AMISA instruction — whether at European Fitness Center (2999 South Virginia Street, 2nd Floor Studio, Reno, NV 89502) or at any school-based or outdoor program location — I authorize AMISA instructors to seek or facilitate emergency medical care for the Participant when I cannot be immediately reached. I accept full financial responsibility for all associated medical costs.
I authorize AMISA to share necessary emergency information (including this document) with emergency responders and medical providers as required to render care.
PART 9: MINOR PARTICIPANT PROVISIONS
If the Participant is a Minor (under 18 years of age), the following additional provisions apply:
• The signing Parent or Guardian represents that they have full legal authority to execute this Waiver on behalf of the Minor.
• The Parent/Guardian accepts responsibility for the Minor's compliance with all AMISA program rules, safety protocols, and instructor directives.
• The Parent/Guardian is solely responsible for timely drop-off and pick-up at all program locations, including school sites and European Fitness Center.
• At school-based sites, AMISA's supervisory responsibility is limited to designated program hours and approved instructional spaces. AMISA is not responsible for Minors outside those boundaries.
• The Parent/Guardian must document all authorized pick-up persons with AMISA in advance. AMISA may follow school site release protocols where applicable.
• The Parent/Guardian acknowledges that school-site programs operate under school and district policies that may impose additional requirements.
